PortaCount Request Process


VHEPC has 3 coalition-funded PortaCount machines that can be distributed to coalition members for the purposes of supplementing coalition members’ existing fit testing capabilities.

The overall request process has 4 main steps:

  1. Coalition member submits a request form and agrees to terms and conditions

  2. Request form processed; Coalition member notified of next steps

  3. Check in machine to facility: Coalition member completes agency to agency transfer of PortaCount machine

  4. Check out machine from facility: Coalition Member completes agency to agency transfer to next Coalition member


Request Form

To request a PortaCount machine, submit a request using the form below.

NOTE: Submission of the request form does NOT guarantee a PortaCount will be sent to your facility on your preferred dates. PortaCount use is subject to the availability of the PortaCount assigned to that region of Vermont.


Notification

Once your request is scheduled, you will be notified via email by VHEPC Readiness and Response Coordinator with the email on file in your request. Facilities are responsible for transportation however VHEPC can reimburse for costs.
